The Joker has been proven capable of improvising on the spot, and Dent's coin being on the table suggests that it has value to Dent (Dent was also seen reaching fo the coin, which Joker may have seen while in his nurse disguise). It's possible Dent's coin-flip (before Rachel's death) was well known, but I'm not sure if Joker knew about it beforehand. However, what I am relatively certain of is that Joker was not expecting Dent to start using a "coin flip execution". Dent seems to have thought of that on the spot.

When the Joker says to Dent "you know the thing about chaos - it's fair", I thought he was urging Dent to use the coin to decide the fate of the others. He seemed to have done some homework about Dent. Did I misread it?
