-1 because this seems to contradict itself... "Everything she did or was doing was on Silva's command" but then "fear of Silva and hope that Bond just might be her savior, brings her to do what she did." Also it ignores key scene - see my answer.

I agree it's more satisfying to have an in-movie answer. But movie content is frequently impacted by logistical concerns. For example, on director's commentaries on DVDs, you'll often hear "we wanted to do X but couldn't because of Y."

Wow, how could they cut such a crucial element from the movie? IMHO the film would have benefited greatly if there was a more-in-depth exploration of the dream/reality question. Instead, it feels like this element of the story is barely touched.

In the original movie, the man's drop of sweat indicated the man was not a computer-generated hallucination but a real human who was scared because he was in real danger. Similarly, in the remake, the woman's tear indicated she was experiencing genuine emotion and therefore was a real person. I'll edit this into the answer now...
