@cde I suspect he introduced himself as Tyler. Marla's perplexed expression and reaction when he told her to say his name wouldn't make sense if there was actual confusion about his name.

Side note: the doctor doesn't refer him to support groups in lieu of medication to help with his insomnia. Jack claims that he's "really hurting" so the doctor sarcastically tells him where he can meet people who are really hurting. In effect, "You're not hurting and you don't need meds. Go chew some valerian root and stop bugging me."

No other characters see/react to Tyler Durden, so he is the only thing in the movie that is part of the Narrator's delusion. If Dr. Railly is delusional--with all the police, doctors, and other characters interacting with Cole--the entire movie becomes a delusion, and we're left with nothing real.

Even when they did send him to the correct time and place (Philadelphia and Baltimore), he appeared above ground and in different locations. So clearly space travel is required even if you assume it automatically adjusts for Earth's continuous motion through space.
